Patient-specific quality assurance strategies for synthetic computed tomography in resonance-only radiotherapy of the abdomen

Background and purposeThe superior tissue contrast of magnetic resonance (MR) compared to computed tomography (CT) led an increasing interest towards MR-only radiotherapy. For the latter, dose calculation should be performed on a synthetic CT (sCT). Patient-specific quality assurance (PSQA) methods have not been established yet this study aimed assess several software-based solutions.Materials methodsA retrospective was 20 patients treated at MR-Linac, which were selected evenly cover four subcategories: (i) standard, (ii) air pockets, (iii) lung (iv) implant cases. The neural network (NN) CycleGAN adopted generate reference sCT, then PSQA methods: (A) water override body, (B) five classes with bulk densities, (C) sCT generated by separate NN (pix2pix) (D) deformed CT.ResultsThe evaluation endpoints demonstrated that while all A-D provided statistically equivalent results (p=0.05) within 2% level for standard cases (i), only C-D guaranteed same result over whole cohort. densities shown valuable method in absence beam path.ConclusionThe observations suggested use additional appropriate tool perform workflow MR-Linac. time requirements respected, namely 10 minutes 2%.
